Established in 1984, Hypertension Nephrology Associates, PC, is a diverse, privately held practice in Livonia, Michigan, serving the Detroit metropolitan area. Board-certified in nephrology, hypertension, and internal medicine, the skilled physicians take an integrative approach to health care, providing care for people aged 20 and older.

Hypertension Nephrology Associates, PC, provides specialized medical management of acute and chronic kidney disease and hypertension, plus dialysis and post-kidney transplantation care.

The providers offer office consultations and follow-up services to patients with less critical conditions who require a nephrology specialist, like people with diabetes, electrolyte disorders, and hematuria.

The practice also provides diet and nutrition guidance as well as education about lab values like glomerular filtration rate (GFR), which is a measure nephrologists use to assess kidney function.

Our New Partnership Creates a Specialized Program for Patients with Kidney Disease

HNA has partnered with Evergreen Nephrology, a national nephrology group that focuses on fostering a holistic-care experience for individuals living with chronic kidney disease to address the needs of our patients suffering from kidney disease. Through this partnership, HNA will gain access to innovative tools and additional manpower to pay close attention to the special needs of patients in a new program called the Complete Kidney Care program.

In the upcoming months, our team at HNA will work with Evergreen to identify patients in our current population who will qualify for this new program. At the request of your HNA kidney doctor, a representative from HNA or Evergreen may contact an eligible patient to enroll them in the program and create a personalized care plan tailored to the patient’s conditions, medications, diet, and social needs.

These services provided to eligible patients are covered by insurance! Patients with deductibles, co-insurance, or copays, may still be expected to pay that portion, but these services will not impact your health insurance premium.
