Chronic conditions are those that are chronic, meaning they are sort of here to stay. Living with any chronic condition can be challenging. We find ourselves restricted from doing the things we used to do, eating the things we love, and meking lifestyle changes we never expected or wanted. This forced change and leave most of us feeling frustrated, confused, and for some, depressed. At HNA, we understand the challenges our patients face so we've made it our mission to provide as many resources as we can to help them, and their caregivers, navigate their way through the challenges of managing the chronic conditions. Our goal is to keep you from experiencing unexpected changes in your health and give you the tools and support to meet this goal.
At HNA, each of our physicians has a set of designated team members to ready to assist their patients with dedicated support in our Care Management programs. Our Care Managers are here to support the patient through their journey each step of the way.
Established patients will be referred by their provider to the Nurse Care Manager. The Nurse Care Manager will review all medical records available and the provider's desired care plan. They will then contact the patient by phone to introduce themselves, remind them their provider referred them to Care Management, and explain the support they will provide.
The Nurse Care Manager is the patients personal coach and health mentor. Their goal is to keep you from experiencing unexpected changes in your health and give you the tools to meet this goal.
They will contact their patients regularly in-between visits to help keep the care plan on track. They will oversee care coordination activities, such as transition care from hospital to office, to dialysis or transplantation. They will help with medications, from understanding the reason for a medication to be taken, changed, or stopped. They will help educate their patient on treatments prescribed and proper diets.
The patient must be established.
The patient must have seen their HNA provider once within a 12 month cycle. The provider must be referred by their HNA provider, who may do so if they have seen the patient within 12 months.
The patient must provide consent either verbally to their Nurse Care Manager or in writing.
Care Management support is for everyone! Our kidneys are critical to our systems. You owe it to yourself to take the time to learn why and what you can do to manage the new challenge.
